A Community of Care 

 Our comprehensive approach to low vision care integrates multidisciplinary services in optometry, occupational therapy, psychological counseling and clinical research. Our patients enter our continuum of care by first consulting with a low vision rehabilitation optometrist who evaluates eye health and visual function to provide the patient with information about their specific eye condition. Each optometrist will then design an individualized rehabilitation plan with our team of low vision professionals that may include recommendations for vision devices, assistive technology, psychological support and/or supplementary Lighthouse services.

SCHEDULE:                As contracted, 8:45-5pm M-F 

LOCATION:                1850 W Roosevelt Rd. Chicago, IL

STATEMENT OF PURPOSE: Provides comprehensive vision rehabilitation care and eye care to Low Vision Clinic patients, agency consumers, and their families as needed.

RESPONSIBILITY FOR THE WORK OF OTHERS: 

This position has no direct supervisory responsibility.

TRAVEL REQUIRED:   Travel within greater Chicagoland area to Chicago Lighthouse satellite clinics (including Indiana) may be required up to 50%.
